Charlestown Community Quality of Life Survey
CHAD encourages everyone to complete the Quality of Life Survey. The survey only takes 10-15 minutes and will provide much needed information to Spaulding and MGH regarding health issues in Charlestown.
The MGH Center for Community Health Improvement is partnering with Spaulding Rehabilitation Hospital to work with the community of Charlestown to determine the most important health issues affecting the residents of Charlestown and develop a plan to work on them. A community assessment committee, which includes many organizations, residents and service providers in Charlestown, has been formed to help guide this process.
We need to hear from you! Over the next two months, we will be talking to residents and collecting data through surveys and meetings. In the spring this data will be presented at a community forum where you will have the chance to determine what issues Charlestown should work on and start to develop a plan to address them. A final report will be created which will be shared with the community in September.
